Medical Coverage For Utah’s Endurance Events. 5K To 100-Mile Ultra. Obstacle Course. Trail Ultra.

High-volume patient contact is the defining challenge of endurance events — hundreds of runners, thousands of crowd, heat injuries, cardiac events. Guardian MedX covers 5Ks, road marathons, trail ultras, and obstacle events (Spartan, Tough Mudder, Rugged Maniac style) with ALS-led, physician-directed coverage.

The Risk Profile

High Volume. High Acuity. Narrow Windows.

Endurance events are uniquely demanding: a 5K has a narrow 40-minute acute window, a marathon generates hundreds of minor patient contacts plus a handful of critical cardiac and heat emergencies, and obstacle events add trauma to the heat-and-dehydration profile. The response system has to scale.

🫀

Cardiac Events

Sudden cardiac events spike at finish lines. ALS with AED and rapid defibrillation at finish is the lifesaving baseline.

🌡

Heat & Hydration

Summer races produce mass heat illness. IV rehydration, cooling protocols, and active crowd monitoring required.

📊

Patient Volume

Hundreds of contacts in hours. Triage, patient flow, and documented aid station throughput determine outcomes.

Race Documentation

Race organizations and USATF-sanctioned events require specific medical documentation. Our ePCR and AAR satisfy these requirements.

Why This Service Is Different

Three Things Standard Vendors Miss.

The event profile here demands a specific combination of speed, clinical capability, and event-literate personnel. Here’s where most vendors fall short.

/ 01

High-Volume Patient Flow

A mid-size marathon generates 200+ medical contacts in under 4 hours. Without a patient flow system and triage protocol, aid stations collapse and critical cases get missed.

/ 02

Cardiac Risk

One of the highest cardiac-event rates of any event type. AED placement, rapid response, and ALS capability at the finish line is not optional — it’s where lives are saved.

/ 03

Obstacle Trauma

Spartan / Tough Mudder-style events add wet, muddy, awkward-landing trauma. Lacerations, fractures, aspiration. Clinical capability has to match the course.

The Guardian Protocol Applied

Four Stages. Matched To Your Race Needs

Every engagement runs through the same four-stage protocol — translated specifically for this event type. Every stage produces a document your team and your underwriter can read.

01

Assess

Course map review. Aid station placement optimized for risk. Hospital capability mapping. Heat index and weather protocol activation thresholds set.

02

Deploy

ALS + AED at finish line. BLS team staffing by race size formula. Aid station placement every 2 miles (road) or 3 miles (trail). Mobile UTV rover for remote sections.

03

iNTEGRATE

Race HQ medical liaison. Hospital pre-notification for anticipated transports. Radio channel integration with race marshals. Physician-authored heat and cardiac protocols active.

04

Document

Every patient contact in ePCR. Aid station throughput tracked by hour. AAR with trend data delivered within 24 hours. Race-specific insurance documentation provided.

Our previous vendor would have called 911 and waited. Guardian MedX treated and recovered the patient in under thirty minutes. The runner sent us a handwritten thank-you note two weeks later.

Race Director Utah Marathon · 20244

What I appreciate most, beyond the medicine, is that they understand the culture of ultrarunning. They do not pull a runner off the course unnecessarily, but they will not allow a runner to continue when continuing would be dangerous, and they make those calls with a steady judgment that runners and crews respect

endurance & marathon FAQ

Questions Promotors Ask.

Q · What's the right medical staffing ratio for a marathon?

Risk-tier dependent. Guardian MedX uses a formula based on registered runners, heat forecast, course profile (road/trail), and cardiac screening. Baseline for a 2,000-runner marathon: 1 ALS team at finish + 1 BLS team per aid station + mobile UTV rover + GMMC treatment hub. We’ll provide exact staffing in our PEP.

Q · Are you Utah Code 53-2d-210 compliant?

Yes. Guardian MedX operates under an in-house Emergency Physician Medical Director — the same model used by municipal 911 systems. Clinical protocols are owned and maintained by a licensed emergency physician. Utah raised the bar in May 2025; we already meet the full standard, and the audit trail is handled so you don’t have to manage it.

Q · Do you handle obstacle course events like Spartan or Tough Mudder?

Yes. Obstacle events are a specific specialty. We have course-familiarity with all major obstacle brands, mud-aspiration protocols, wet extrication gear, and the clinical capability to handle the trauma-plus-heat combination these events produce.

Q · Can you provide course-side AED coverage?

Yes. Guardian MedX deploys AEDs with every BLS team. Finish-line ALS has additional defibrillation capacity. AED battery and pad expiration tracked as operational KPI. Usage documented in ePCR.

Q · How do you handle mass heat illness events?

Physician-authored heat protocols. IV fluid resuscitation on course. Active cooling at GMMC (chilled fluid, cold pack application, ambient cooling). Race control gets real-time heat illness contact counts so pace / cutoff decisions can be made with data.

Q · Will your documentation satisfy USATF / RRCA sanctioning?

Yes. Our standard PEP and AAR formats meet USATF and RRCA medical coverage documentation standards. Additional sanctioning documentation available on request.
