Five sections, twenty-one Yes/No items covering physician oversight, personnel credentials, documentation, insurance, and operations. A vendor who declines, deflects, or returns it with a single No is not Utah Code 53-2d-210 compliant — regardless of price. This one’s free because the more it circulates, the better the entire industry gets at vetting.
